Clinical Nurse Consultant/Specialist

Our Health Service is expanding!

We are looking for a Clinical Nurse to join our PICS team in a temporary full-time contract until 22 June 2025.

Preventative Integrated Care Service (PICS) provides rapid access (review within 24-48 hours) to intensive specialised medical management, delivered by a team of medical, nursing, and allied health clinicians to adults living with high-risk chronic conditions, to improve quality of life, and reduce length of stay, unplanned hospital presentations, and Potentially Preventable Hospitalisations (PPH).

About the role

The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners and other agencies (including non-Government agencies). This role provides the delivery of clinical care and support to the Preventative Integrated Care Service within the Preventative Health Division.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated advanced clinical knowledge in the area of the chronic condition management delivering expert nursing care in complex situations while providing support and direction to other team members.
  • Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
  • Demonstrated ability to communicate effectively and maintain a positive workplace culture through role-modelling well developed consultation, liaison, negotiation and conflict resolution skills, engaging professionally with members of the healthcare team, patients, relatives and visitors.
  • Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use Virtual Care, digital health/information and reporting systems appropriate to the position.
