  • “Stream” The Integrated Care Service is similar to an urgent care service – 0800 to 1630 Mon-Fri.
  • Supporting ED avoidance and hospital admissions by diverting and redirecting lower acuity patients from the Emergency Department to Integrated Care. .
  • The SMO will work within the previously provided roster, alongside a PHO and Nurse.
