We are seeking a Clinical Nurse for our Community Palliative Care program, where you will conduct comprehensive clinical assessments and collaborate with healthcare providers, clients, and families to create individualised care plans. You will also support team members in implementing these plans and advocate for client choice to receive care in the most suitable environment within an acute/sub-acute/post-acute care framework.

About us:

Community Acute and Non-Acute Services is a multidisciplinary team that focuses on evidence-based practice, continuous quality improvement, risk management, including information-based decision making as mechanisms for improving the quality, standards and safety of the health service provided. CANAS aims to provide the best patient care possible, at the appropriate time in the appropriate place. The principal objective of CANAS is the substitution and avoidance of hospital admission or the facilitation of early discharge to enable restoration of functional capacity. The services offer care across a broad range of clinical conditions and age groups.

CANAS provides two models of care: a hospital substitution model through Hospital in the Home (HITH) service and a hospital avoidance model through multiple, interfaced programs. These focused service areas are: Post-Acute care (PAC), Community Palliative Care (CPC), Complex Care Coordination and a Cognition Service. These are ambulatory, transitional services to assist the patient through the tertiary, secondary and primary (both directions) care journey. 

About the role:

An opportunity exists within our Community Palliative Care (CPC) program. The Clinical Nurse Community Palliative Care (CNCPC) will provide comprehensive clinical nursing assessment and liaise with relevant health care providers, client and family to develop a plan of care and provide expert clinical support to other team members to implement the plan. The CNCPC will promote strategies that will enable client choice to be cared for in the most appropriate environment within an acute/sub-acute/post-acute care model.

The CPC program provides support to residential aged care facility residents by supporting facility staff via a consultative, educational and solutions focus methodology to appropriately manage residents on an end-of life pathway, in collaboration with General Practitioners. The principal focus is to provide the necessary palliative support to residents that would otherwise require hospital-based care (acute, sub-acute) in a familiar environment.

About you:

  • Mandatory qualification/s and professional registration requirements with the Australian Health Practitioner Regulation Authority (AHPRA).
  • While not mandatory, a relevant post-graduate qualification would be well regarded.
  • Demonstrated ability to deliver a high-quality service with the primary focus on achievement of organisational goals for the good of others.
  • Competence in the application of clinical problem solving and effective coordination of patient centred care at an advanced level.
  • Knowledge of quality improvement processes and activities and ability to apply validated research in ongoing improvements of clinical practice.

What we offer:

Total monthly remuneration value of up to $5,495 is comprised of:

  • An hourly rate between $54.44 - $58.30 p.h.
  • 12.75% employer superannuation contribution
