The Diabetes Education and Health Assessment Unit offers a broad range of services for patients with diabetes within the NSLHD. These services include diabetes education and diabetes management for patients with Type 1, Type 2 and diabetes in pregnancy. Young adults with Type 1 diabetes, in particular those young patients transitioning from paediatrics to the adult setting, are a specialty of the unit. The unit has a multidisciplinary team which involves close liaison with allied health, specialist endocrinologists and complication assessment to ensure a rounded package for the patients with diabetes.
What you'll be doing
The "Diabetes Educator RN" will work within the RNSH Diabetes Education and Assessment Service. The Diabetes Educator RN will provide best practice, evidence based care for all patients in the community with or at risk of diabetes.Furthermore, expert advice will be provided to carers and health care professionals as required to:
- Enable the person with diabetes and significant others to minimise the effect of diabetes on daily living, lifestyle, occupation and recreation.
- Enable the person with diabetes to develop practical diabetes self-management skills.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency ( AHPRA )
- Demonstrated interest in diabetes education and management
- Demonstrated commitment to ongoing professional development.
- Evidence of and commitment to patient/customer focused care.
- Demonstrated excellence in written and verbal communication skills and computer literacy in Microsoft applications.
- Demonstrated ability to work well within a multidisciplinary team.
