Join our dynamic, multi-disciplinary team in the beautiful Great Southern region as a key member of our Population Health Directorate! In this rewarding role, The Clinical Nurse Specialist – Community Health is responsible for the coordination of primary health care for vulnerable children and families within the community setting, collaborates and advocates with key stakeholders within and outside the region. Collects data and prepares reports for specific child health projects. Acts as a role model and consultant to nurses and other health service providers providing knowledge, clinical expertise and best practice within area of responsibility. The Country Nursing & Midwifery incentive program is available to new and existing nurses and midwives - across a broad range of patient-facing roles - on fixed term or permanent contracts at eligible locations. Eligible nurses and midwives can receive between $5,000 and $17,000 in support over 12 months. About You
- Postgraduate qualification in child and community health.
- Advanced interpersonal and communication skills (written and verbal) in a multidisciplinary team.
- Clinical knowledge and skills in the area of specialty.
- Knowledge of Primary Health Care principles, Determinants of Health, and family-centered practice.
- Understanding of best practice principles and quality improvement activities.
- Eligible for or in possession of a current C or C-A Class driver’s license, with the ability to travel.
- Awareness of Aboriginal and multicultural health issues.
- Experience in rural or remote environments.
- Proficiency in computer, IT, and Telehealth systems.
- Commitment to Equal Opportunity, Disability Services, and Occupational Safety and Health principles.
- Maintain competence in line with national standards for Registered Nurses and Midwives.
- Provide clinical leadership, coordinating care for vulnerable children and families.
- Develop systems to ensure at-risk children receive appropriate care or specialist support.
- Engage with stakeholders within and outside the region to manage vulnerable children.
- Promote multidisciplinary decision-making and innovative problem-solving.
- Lead quality improvement initiatives and ensure compliance with relevant policies.
- Contribute to program evaluation, research, and evidence-based practices.
- Mentor and support nursing students and team members.
- Ensure the safe, efficient, and effective use of resources.
