What you'll be doing
The Palliative Aged Care Supportive Service (PACSS) includes specialist Nurse Practitioner/Transitional Nurse Practitioners who work across the district to provide support and care for patients who are transitioning between acute care and aged care. The team helps ensure patients have a smooth transition from the acute care setting to the aged care setting. Working closely with GPs, Geriatric Outreach Services, and Residential Age Care staff, to provide patients with the care they need in the residential aged care setting.
The Clinical Nurse Consultant (CNC) is an expert nursing who will provide leadership, advice and recommendations on all aspects of nursing and consults on matters relating to the safe and appropriate use of nursing practices.
The CNC will formulate and implement policy, guide practice development, and engage in education and research activities. The CNC is responsible for the provision of clinical supervision to nursing staff and acts as a role model, providing nursing care on a regular basis.

Candidates will need to meet the following criteria:
- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum five years full time post graduate experience with 3 years full time equivalent in the specialty field. In addition the applicant must have an approved post graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters level qualification.
- Demonstrated clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
- Demonstrated effective communication and interpersonal skills across a variety of mediums and an understanding of the practice development framework.
- Demonstrated ability to undertake quality improvement and research activities that influence and enhance clinical practice and implement change.
- Demonstrated ability to work effectively both as a member of a multidisciplinary team and independently.
