The Central Coast Local Health District (CCLHD) is seeking an experienced Senior Occupational Therapist (Level 3) for our Complex Discharge Advisory Team (CDAT). This role involves collaboratively working with a Social Work colleague and the Multi-Disciplinary Team (MDT) to facilitate complex problem solving for patients and clients engaged with the NDIS, NCAT and where escalated discharge processes are appropriate. You will be responsible for a Check, Chase, Challenge model of support to the acute services including capacity building within Allied Health and other members of the MDT.
