Nurse Practitioner

Neonatal Services are looking for a Nurse Navigator to join their team. The role supports advanced neonatal care by coordinating patient-centered services, fostering partnerships, and improving outcomes through Nurse Navigator principles. It enhances complex patient care, boosts system efficiency, and reduces costs by streamlining and coordinating the care process, ensuring a safe and effective patient journey across the continuum of care.

About the Role

  • The role requires that the Nurse Navigator participates in reflective clinical practice, mentorship/clinical supervision monthly.

  • Exercise advanced clinical judgement, informed by an evidence-based model using critical thinking and ethical decision-making processes, in the provision of patient-centred care coordination across the patient journey.

  • Provide a central point of communication, engagement and coordination for key partners involved in the individual patients care across primary, secondary and tertiary care providers utilising multi-modal communication pathways facilitating the patient focused care journey.
  • Advocate effectively through the provision of skilled nursing consultation within healthcare team decision-making forums and proactively addresses enablers and barriers impacting on the role.
