About the role
Our single goal is to provide a service that builds'healthier rural people and thriving communities', and in this role, you’llgrow long-term relationships with patients, simplifying referral pathways and access to care to help them manage their mental health within their own communities.

This role undertakes mental health assessment, crisis intervention, acute assessment in Emergency, care planning and case management for adult clients experiencing mental health problems and psychological symptoms.

Providing outreach to Grenfell and Canowindra as required. Improving the quality of life for people with severe and complex mental health needs requiring intensive intervention in a community setting.

This position is based in Cowra - with flexibility to work from other sites within the Western NSW Local Health District.
