About the Opportunity
An opportunity exists for an interested Registered Nurse to join the Bowral team as Clinical Nurse Specialist - Grade 2 - Transfer of Care and Discharge Facilitator.

This exciting role incorporates utilising predicative measures for planning patient flow, family meeting co-ordination, MDT co-ordination for discharge planning and Journey Board lead. If this interests you please apply to become part of our nursing team in the beautiful Southern Highlands

What You'll be Doing
The Clinical Nurse Specialist Grade 2 will provide safe, compassionate, quality, person-centred specialist care and work in collaboration with patients/families/carers and a multidisciplinary team to achieve identified goals and outcomes.


This role ensures efficient and effective co-ordination from admission to discharge of patients with the aim of effecting an improved patient experience as well as access and flow within the hospital. This role works in conjunction with the Operational Nurse Manager to co-ordinate resources that operationally impact on patient flow from patient admission to discharge and institutes practices aimed at improving discharge times and processes optimising bed availability.

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

  1. Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA), and 3 years relevant Full Time equivalent post registration experience, and approved post graduate nursing/midwifery qualifications in a field relevant to the specialty field, or such other qualification, or experience deemed appropriate by the organisation with highly developed computer literacy in Microsoft Office applications and health support platforms including Cerner, EMR2, Patient flow portal.
  2. Demonstrated knowledge of Whole of Health priorities and skills in clinical co-ordination and patient flow initiatives
  3. Demonstrated ability to provide safe compassionate quality care that is culturally responsive
  4. Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs
  5. Demonstrated capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise
  6. Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based nursing care
  7. Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace
  8. Demonstrated understanding of operational factors that influence patient access and improve patient journey and impact on capacity management
  9. Proven leadership qualities and the ability to set priorities, establish and achieve agreed objectives and the ability to work with a broad range of health discipline
