An exciting opportunity is currently available for an experienced Clinical Nurse (Case Manager) to join the Integrated Care Rehabilitation Team at the Bundaberg Hospital within the Wide Bay Hospital and Health Service. The Wide Bay area is arguably one of the most beautiful regions of Queensland. The Health Service stretches from Agnes Water in the North to Maryborough in the South and provides acute care services to a population of over 225,000 residents including several rural communities.

Eligible Applicants who move to Queensland from an interstate or international jurisdiction to take up permanent employment may receive up to $20,000 incentive payments under the Queensland Health Workforce Attraction Scheme

Your opportunity:

  • The Transition Care Program is an exciting jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria.
  • The position of clinical nurse (Case Manager) will have responsibility for the assessing, care planning, provision of nursing care, coordinating and monitoring of allocated clients within the Transition Care Program.

Our offer:

You will receive a competitive remuneration package value up to $92,330 p.a., comprising salary rates $54.45 - $58.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Permanent Part Time, 53.2 hours p.f.).
