About the role
The Senior Social Worker - Virtual role is an integral member of the newly established Western NSW Local Health District Patient Flow & Access Command Centre. This position will lead essential collaboration with fact-to-face and virtual services to provide a multidisciplinary approach to care for positive outcome for patients with complex care needs.

The Senior Social Worker position sits within the Western Virtual Division, as part of a multidisciplinary patient flow and access team. This is a highly collaborative team that continually strives to provide comprehensive patient-focused care with multi-disciplinary teams physically located across the Health District.

The right person will be able to work independently to provide clinical leadership and high-level expertise to proactively direct care for patients with complex care needs.

The role offers, flexible location with hybrid options for work, flexible work hours, working with rural communities and highly collaborative teams.
