What your day will look like….
Each day, you will be at the heart of patient care coordination, liaising with medical, nursing, and allied health professionals to ensure a smooth transition from hospital to home for our patients. You'll oversee discharge planning, managing the logistics, timelines, and care needs, all while fostering strong communication with patients and families. Your critical thinking and advanced clinical judgment will play a key role in resolving complex discharge scenarios, ensuring patients are supported with the right care pathways post-hospitalization. You'll also guide and mentor fellow team members, contributing to a culture of continuous improvement and high-quality care.
