Nurse Practitioner

The RaSS/GEDI Service operates seven days, with extended hours. The service provides care to frail older persons in the Emergency and Community settings including RACFs. Specialist gerontic nursing care supporting the general function of the unit through the application of the principles of best practice, education, leadership and portfolio management. The RaSS/GEDI team operates collaboratively with community-based services, residential aged care facilities, hospital inpatient teams, hospital in the home services and multidisciplinary Allied Health services to provide evidence-based care to clients who meet specific criteria. The RaSS/GEDI team also provides care in the community to patients by reviewing those with acute deterioration in the Residential Aged Care Facilities (RACF), who would otherwise be sent to Emergency Department.

About the role

The Nurse Practitioner (NP) – RaSS/GEDI is a Registered Nurse who demonstrates advanced and expanded clinical skills, knowledge and high level of capacity to translate these into contemporary evidence based emergent clinical care for residents in a Residential Aged Care Facility (RACF).

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated contemporary expert nursing clinical knowledge, expertise and skills in nursing practice and leadership enabling the delivery and supporting high quality care provision in emergency care to the level required within mandatory qualifications section.
  • Demonstrates the ability to apply specialised knowledge of contemporary advanced nursing practice and theory in emergent and geriatric nursing and to participate directly and indirectly in the delivery of clinical care.
  • Demonstrates the ability to build and maintain strong relationships, open communication and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
  • Demonstrates the ability to educate, inform, counsel and support patients and families with contemporary clinical information at an advanced level relevant to patient's care and understanding.
  • Applied knowledge of quality improvement frameworks and associated quality processes inclusive of the integration of nursing research, identifying experiences that evidence a professional commitment to contributing to quality improvement.
  • Demonstrated ability to interact professionally and maintain professional relationships with all members of the healthcare team both internally and externally to the health service.

Mandatory Requirements:

  • Registration as a Registered Nurse with Endorsement as a Nurse Practitioner with AHPRA and a current annual practising certificate are mandatory for this position.
  • Ability to meet credentialing requirements of WMH is mandatory for this position.
  • Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
