The role
In this role, you will deliver high-quality, evidence-based nursing care within a specialist area such as Chronic Disease Management, cardiac rehabilitation, diabetes education, or respiratory rehabilitation. Working as part of a multidisciplinary team, you will provide direct patient care, support system-wide health initiatives, and contribute to a culture of continuous improvement. Responsibilities include using contact centre-specific technology to support clinical decision-making, educating patients and caregivers to promote health independence, and ensuring compliance with Queensland Health standards, policies, and procedures. You will also uphold workplace safety, participate in quality assurance processes, and engage in professional development to enhance clinical skills.
This position requires accountability as an independent and interdependent practitioner, reporting to the Nurse Unit Manager via Clinical Nurses. The role involves contributing to research, education, and service promotion, including delivering education sessions to referral sites across Queensland. A commitment to teamwork, open communication, and professional leadership is essential, as is the ability to adapt to a dynamic work environment, including potential remote work opportunities. Flexibility in work hours may be required to meet service demands, along with participation in activities supporting the recruitment and retention of program participants. Registration with the Nursing and Midwifery Board of Australia, effective computer skills, and the ability to work across nursing services within the Health Contact Centre are essential.
