Come Join the Care Awaiting Placement South (CAPS) Team as a Nurse Manager!
CAPS is a time limited, short term/temporary stay in an aged care facility for patients awaiting permanent residential care.
The focus whilst patients are on CAPS is to assist them to find a permanent residential care placement with assistance from the placement officers and Social Work. The placement officers who have commenced working with the patient and family in hospital continue to assist in securing permanent placement when the patient moves to a CAPS bed.
About the role:
Reporting to the Transition Services Manager, the CAPS Nurse Manager provides clinical nursing expertise for the triage, assessment, coordination and acceptance of patients to Care Awaiting Placement South (CAPS). The CAPS Nurse Manager also oversees the monitoring of these patients whilst on the CAPS program with close liaison with key stakeholders. The Nurse Manger provides line management, coordination and leadership of nursing and/or multidisciplinary team activities to achieve continuity and quality of patient care outcomes.
Duties for the role will include:
- Provide the pivotal leadership and co-ordination of patient/client care delivery in a defined ward/unit/service/program to achieve continuity and quality of patient/client care and outcomes and efficient patient/client flow,
- Contribute to the development of, implementation of, and monitoring of corporate policies and processes and lead in their area of expertise.
- Ensure mechanisms are in place to support ongoing education where work and learning are integrated,
- Uses metrics and research outcomes to identify the need for future evaluation or research action in order to improve practice and service delivery,
- Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.
About you:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate,
- Registered Nurse and or Midwife with at least 3 years post registration experience,
- Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience,
- Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills,
- Ability to review and improve models of care to be person and family centred.
