Lotus Glen Health Service (LGHS) is a rapidly growing division within the CHHHS region, delivering primary health care services to detained, remanded, and sentenced prisoners located at the Lotus Glen Correctional Centre (LGCC) and Lotus Glen Open Custody Farm. The Secure Centre has a capacity to house approximately 976 prisoners and the Open Custody Farm accommodates 124 prisoners. The main catchment area of the centre is North Queensland, with a disproportionately large representation from first nations communities. Nursing cover is provided 24/7 with nursing staff responsible for patient assessment, minor treatment, and patient transfers.

About your opportunity:

In this position you will provide comprehensive, holistic nursing services to patients within the Cairns and Hinterland Hospital and Health Service.
The Clinical Nurse is responsible for a portfolio with a focus on clinical care/leadership, management, education and/or research. This role also provides support to Nurse Grade 7 or above in management activities.
As a Chronic Disease Clinical Nurse at Lotus Glen Correction Centre, you will provide clinical management and leadership for patients with chronic diseases within a correctional setting. Your role includes coordinating high-quality, culturally sensitive care, developing individualized treatment plans, and ensuring patient safety and satisfaction. You will lead and mentor nursing staff, collaborate with multidisciplinary teams, and implement continuous improvements in care delivery in managing complex health needs and enhancing patient outcomes.

About your role:

  • Conduct comprehensive assessments for individuals with chronic diseases, including developing and updating individualized care plans.
  • Deliver evidence-based interventions and treatments, ensuring the highest standards of care and safety for patients with chronic conditions.
  • Facilitate and coordinate care among multidisciplinary teams, ensuring seamless integration of services and effective patient management.
  • Develop and maintain strong partnerships with Hospital and Health Services, Queensland Corrective Services, and other relevant stakeholders to ensure integrated care delivery.
  • Assist in the creation and implementation of policies and procedures related to chronic disease management, ensuring they reflect best practices and organizational values.
