About Your Role

Provide care coordination using an Integrated Care approach across acute and community settings to achieve positive health outcomes for people with chronic and complex conditions.

Work collaboratively with key stakeholders to ensure people with chronic conditions receive appropriate services to attain their health goals and:

Avoid potentially preventable hospital admissions and ED presentations

  • Develop a network to deliver individualised care for people with complex medical and psychosocial conditions.
  • Provide a care coordination service across organisational boundaries integrated with primary care, community, acute and social support services.
  • Ensure people with chronic/complex conditions receive optimum care and support to remain safely in thecommunity.
  • Actively contribute to the Integrated Care Team to ensure equitable access to a flexible, supportive district-wide service.
