About the role
Lismore Community & Allied Health are seeking a Clinical Nurse Specialist Grd 2 to join their Hospital in the Home team. This role provides clinical leadership and specialised clinical practice in an integrated model of nursing care called Hospital in the Home (HITH), which is designed to prevent hospital admissions and support shorter length of inpatient stay. This service is delivered across the community and acute interface and incorporates medical clinical governance, patient and staff safety and operates within the parameters of Local Health District (LHD).
