We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across the acute and sub-acute inpatient setting.
About the Role
The Casey Social Work Department is looking for a Grade 1 social worker to join the team working in the subacute hospital setting including Gem and Rehab programs. This position is a full-time, permanent position and is part of a flexible team who collaborate to provide a holistic approach to service delivery across Casey Hospital, also working collaboratively across all sites bed-based and community.

As a member of the team you are integral to the Monash Health Social Work service delivery and consistently demonstrate our core values. You will be part of a flexible workforce with personalised support. You will be a member of a dynamic team with the opportunity to work in a setting assisting adult patient cohorts from general medicine, general surgery, Emergency department, Women’s and Children’s and other specialty areas as required. The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.
