An exciting opportunity to join a new program providing face to face specialist Palliative Care Nurse Practitioner support 4 days per week (Friday to Monday) for patients, carers and families where timely intervention is required to support patients with distressing symptoms and rapid deterioration.

Where you'll be working
- A Registered Nurse appointed as such to a position approved by the Secretary NSW Health and who is endorsed by the Board, to practise as a nurse practitioner.
- Holds a current NSW driver’s licence, enabling flexibility and mobility for community-based care.
- Proven clinical expertise and competency in the management of patients requiring Supportive and Palliative care, with a track record of delivering high-quality, person-centred care.
- Demonstrated ability to work autonomously whilst effectively contributing to community nursing services and engaging in multidisciplinary collaboration to achieve optimal patient outcomes.
