About You
- You have current authority to practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- You possess demonstrated ability to work effectively and efficiently in a range of mental health Settings with a commitment to contemporary mental health service provision principles.
- You have demonstrated high level of communication, negotiation and interpersonal skills and the ability to work in partnership with consumers, carers and General Practitioners.
Where you'll be working
The position of Registered Nurse, Care Coordination Team, Non-Acute Community Mental Health Service exists within the Central Coast Local Health District. The position is based in Community Health Centres on the Central Coast and may include shift Work at times.
The Non-Acute Community Mental Health Service team is a multidisciplinary team, it provides non-acute assessment and care planning and Treatment for consumers presenting with a mental illness in the moderate to severe range. The position sits within the multidisciplinary team comprising Psychology, Social Work and Nursing. All aspects of service design and delivery are recovery oriented and therefore client-centred and collaborative, family inclusive and trauma-informed. The recovery model assumes that people with complex mental health conditions have the capacity to improve and to live a life that is not defined by their illness.
The Teams liaise closely with local General Practitioners and other service providers to assist consumers to develop sustainable support networks and promote positive mental Health outcomes.
The Registered Nurse is an integral part of the multidisciplinary healthcare team, providing nursing care to patients and consumers of thehealth service; and leading, directing and supervising other nurses and care roles in the team, such as Assistants in Nursing and Enrolled Nurses, to support quality care for patients/ consumers.
