The Community Services Team are looking for a Clinical Nurse to join their supportive team. The Community Services Team is a multi-disciplinary team that provides acute and post-acute care via hospital avoidance and hospital in the home interventions. Care is provided across seven days, in various settings including hospital clinics, community centres and consumers’ places of residence. The team operationally reports through the Rehab@Home stream within Metro South Health@Home, Community and Oral Health Services.

About the Role
- The Clinical Nurse (CN) role provides advanced clinical and problem-solving skills, expert planning, and coordination skills in the clinical management of Community Services Team clients with complex care needs, ensuring quality standards are met
- Has demonstrated knowledge of the social factors impacting on health outcomes and the importance in the application of the Principles of Primary Health Care (PHC) to guide the delivery of clinical care and displays an ongoing commitment to the acquisition of advanced skills in PHC.
- Works autonomously within the healthcare team structure and is pivotal in the planning, managing, and delivering of community specialist nursing care
- Provides care to community clients in either the home or clinic setting to deliver a wide range of Acute (HITH, GEMRHITH patients) and Post-Acute (MAPS - Multidisciplinary Avoidance and Post-Acute Service patients)
