Virtual Rehabilitation Ward has a strong focus on innovation and digital strategies, with Telehealth and home-based monitoring integrated into the provision of care.
About the role:
Working across the Virtual Rehabilitation Ward, this role is integral to supporting high-quality service delivery. Duties for the role will include:
- Managing the clinical nursing team to enable the safe and effective delivery of care to patients with complex medical, functional and/or psychosocial needs within their home environment or equivalent in lieu of a hospital admission.
- Supporting targeted, evidence-based interventions will be key to the success of the service and all team members will be responsible for service outcomes and evaluation.
- Leading, developing and fostering a positive work culture which is based on SA Public Sector and SALHN values and promotes patient / client focussed service, learning and development, safety and welfare of employees, acknowledges differences, and encourages creativity and innovation.
About you:
- To be appointed you must beRegistered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Registered Nurse with at least 3 years, full time equivalent, post registration experience.
- Demonstrated competence in geriatrics, rehabilitation and community nursing practice in accordance with the relevant standards
- Demonstarted ability to work effectively as part of a multidisciplinary team and facilitate change management.
- Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses.
