What you'll be doing
Join our team as an ACAT Assessor, providing vital support to older people through comprehensive assessments and care planning in diverse settings. Your responsibilities will also include:
- Working within a multidisciplinary team to deliver high-quality services.
- Assessing eligibility for Commonwealth-funded aged care services.
- Developing tailored support plans to meet client and carer needs.
- Providing care in both community and hospital environments.
- Contributing to improving aged care outcomes in Western Sydney.

Essential Criteria:
- Tertiary qualification & current registration with AHPRA in Nursing, Physiotherapy, Occupational Therapy or tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Work or Speech Pathology with eligibility for membership of Speech Pathology Australia.
Selection Criteria:
- Demonstrated post graduate experience working with older people in an acute or community setting.
- Demonstrated ability to work independently & in an effective manner encompassing appropriate initiative, organisational, problem solving & time management skills.
- Proven written & verbal communication skills including demonstrated computer literacy.
- Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team.
- Understanding or experience in the health & functional support needs of older people & their carers in the community setting.
- Current Drivers Licence (minimum class C) valid within Australia & demonstrated competent driving ability/ experience.
