About us:
The Sub-Acute Chronic Care Rehabilitation (SACCR) team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unplanned medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care. This service uses the interdisciplinary client centred model as described in the SACCR service profile, with student placement and clinical education embedded in this core business.
About the role:
As part of the SACCR team, the Nurse Practitioner (NP) in Chronic Disease provides specialized care for patients with chronic conditions, aiming to reduce unplanned hospital admissions and support early discharge through community-based care. The NP's role includes advanced assessments, prescribing medications, and coordinating care with the interdisciplinary team. This position aligns with the SACCR service's client-centered model, with 20% of the NP's time dedicated to non-clinical duties, including student learning and clinical education.
About you:
- Current nursing registration through the Australian Health Practitioner Regulation Agency (AHPRA).
- Nurse Practitioner Masters level qualification and NMBA endorsement to practice as a Nurse Practitioner is mandatory.
- Demonstrate advanced and expanded clinical knowledge and skills in the specialty field of chronic disease nursing.
- Demonstrate evidence-based practice, conduct research activities and quality initiatives to improve and inform practice change.
What we offer:
Total remuneration value of up to $177,797 p.a. is comprised of:
- An hourly rate between $75.27 - $78.59 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
