Make every day different by joining our Patient Flow Unit. We have a current vacancy as a Nurse Manager (Interfacility Coordinator) where you will be accountable at an advanced practice level for the coordination of clinical practice for interhospital and intrahospital transfers across the CHHHS.

About you:

In your new role you will:

  • Engage with inpatient multidisciplinary teams (MDTs) to enhance planning for discharge or transfer, applying nursing knowledge relevant to the area of professional practice, problem solving patient flow issues at an advanced level. Assist with and advise on patient transport options with internal and external stakeholders.
  • Demonstrates sound knowledge of contemporary nursing practice and theory and assists with review and recommendations on strategic initiatives to ensure sustainable processes in the management of interhospital flow, inclusive of the development of procedures and processes which facilitate and improve inter hospital transfers between CHHHS, intra-state and inter-state sites.
  • Provide leadership to effectively plan, manage and evaluate health services to achieve organisational goals across the service, whilst providing effective communication, consultation and negotiation throughout the service ensuring the involvement of stakeholders.
