The Central Coast Local Health District is seeking a highly motivated and experienced Social Worker Level 5 to join our team as an Inpatient Site Manager at Wyong Hospital. In this permanent full-time role, you will provide leadership and operational management to our inpatient social work team, ensuring high-quality, patient-centered care that supports optimal health outcomes for patients and their families. With a focus on managing complex psychosocial cases, you will play a pivotal role in delivering trauma-informed, culturally responsive care, while collaborating closely with multidisciplinary teams across the hospital. As the Inpatient Site Manager, you will lead and manage the Inpatient Social Work team, ensuring that services are delivered in accordance with relevant policies, procedures, and best practice standards. Your role will involve providing complex psychosocial assessments, crisis interventions, and case management services to patients and their families, particularly in cases of trauma, grief, loss, and domestic violence. You will work autonomously to address novel and critical situations, offering expert guidance and solutions. In addition to direct service delivery, you will play a key role in team development, providing clinical supervision to junior social workers and facilitating ongoing education and training initiatives. You will also be responsible for overseeing service planning, quality improvement, and the smooth transition of patients from hospital to community-based services.

Your leadership will be essential in fostering a collaborative and supportive environment, ensuring that the social work team is equipped to meet the needs of patients across a variety of clinical specialties. You will actively contribute to the development and evaluation of social work services, advocating for the integration of evidence-based practices and patient-centered care.

About You 

  • You must have a tertiary qualifications in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW).
  • You have advanced experience in psychosocial assessments and crisis management.
  • You have extensive experience in providing psychosocial, risk assessment, and crisis case management services.
  • You have strong verbal, written and interpersonal communication skills, including the ability to adapt communication style to suit different audiences,along with proficiency in using information technology to enhance patient care.

Where You’ll Be Working

This role will be based at Wyong Hospital within the Central Coast Local Health District (CCLHD), a vibrant and rapidly growing region. As an integral member of the hospital’s multidisciplinary team, you will work in a dynamic and fast-paced clinical environment, where you will engage with a diverse range of patients and clinical specialties. The role involves close collaboration with hospital management, senior clinicians, and external service partners to enhance service delivery and ensure the highest quality care for patients. You may also be required to work across other CCLHD facilities as needed, including participation in the Social Work Service After Hours Manager on Duty (AH-MoD) roster. This offers a diverse and varied working experience, allowing you to have a broader impact within the district.
