About the Opportunity
This is an exciting opportunity to lead a new End of Life multidisciplinary team within Acute & Extended Care in Community!
This position is responsible for providing operational and team management and professional leadership to the multidisciplinary team located within the End-of-Life Care team in Primary and Community Health which will provide services across SWSLHD. The team consists of Social Workers, Nurse Practitioners, Clinical Nurse Specialists and Occupational Therapists. In addition, this team will need to work closely with the following teams Palliative Care, Triple I CoPilot, Child Health Nurse and Neurodegenerative.

What You'll be Doing
This position is responsible for providing operational and team management and professional leadership to the multidisciplinary team located within the End-of-Life Care team in Primary and Community Health which will provide services across SWSLHD.
The position will lead and oversee the provision of a multidisciplinary face to face and virtual clinical care service to consumers and their carers for End-of-Life Care and ensure the provision of optimal health outcomes that are consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct.
The position is expected to provide operational management of the multidisciplinary team as well as leadership in quality activities, manage specific projects, and assist in human resource management and other relevant departmental administrative duties.

- Social Work degree and eligibility for the Australian Association of Social Workers (AASW) including the ability to demonstrate a practical application of the AASW Code of Ethics.Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
- Demonstrated experience in providing guidance, leadership and management of a multidisciplinary team.
- Demonstrated understanding of end of life care and the strategies and models that underpin this philosophy
- Demonstrated experience in prioritizing a challenging and demanding workload; managing stressful situations; and applying advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature.
- Demonstrated advanced communication skills (verbal and written), including excellent negotiation, complex decision making and advocacy skills and demonstrated computer skills.
