What you will bring to the role

1. Degree in Social Work and eligibility for membership of the Australian Association of Social Workers. Overseas qualifications must supply the AASW Overseas Assessment number.
2. Demonstrated high level of clinical expertise and knowledge in the assessment and management of clients with chronic and complex conditions including experience in palliative care across community or hospital settings.
3. Demonstrated leadership ability to provide clinical support, education, and consultation to health professionals in area of specialty to improve client outcomes and service design.
4. Demonstrated problem solving and decision-making abilities according to best practice including the ability to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
5. Demonstrated ability to communicate effectively with clients, carers and stakeholders and provide advice to managers on service development.
6. Demonstrated commitment to evidence-based practice and quality improvement.
7. Ability to work independently with minimal supervision and maintaining involvement within the wider interdisciplinary team.
8. Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
