We are looking for a Senior Physiotherapist to join us and deliver acute care in the home as part of a Multidisciplinary Team.


Where you'll be working
Across our Northern Sydney Local Health District - based at Hornsby, Royal North Shore and Mona Vale.

Hospital in the Home (HITH) is a Multidisciplinary team located across our District and provides 7-day cover to deliver acute care at home.
The team includes physiotherapists, occupational therapists, pharmacists, social workers, nurses, doctors, and community care aides.
Together, high quality acute care is delivered flexibly and ensures the patient is the centre of our care whilst at home or in our clinic.

What you'll be doing

As the Senior Physiotherapist, your acute skills in respiratory assessment and treatment are necessary to support our patients at home and avoid hospital admission or support early discharge home.
In addition to this, common cohorts of patients include post orthopaedic (elective and trauma), cardiac (stents), breast surgery and frailty patients who would otherwise be in hospital.
Responding to acute deterioration in the home and escalating appropriately is a frequent requirement of this role and supported by the HITH team.

Candidates will need to meet the following criteria:

  1. Full registration with the Physiotherapy Board of Australia. Evidence of extensive clinical experience with a minimum at a Level 2 Physiotherapist. Hold an unrestricted NSW Driver's License.
  2. Provide evidence of proficient written, verbal and interpersonal communication skills with the proven ability to liaise with novice and experienced health professionals and external service providers. Demonstrated computer literacy and ability to use health information systems.
  3. Demonstrated proficiency in ability to work autonomously with limited supervision. Have the ability to prioritise work, manage a demanding workload, apply high level reasoning skills and have independent professional judgement.
  4. Demonstrated proficient skills and knowledge in the provision of current physiotherapy practice and understand the Model of Care for Hospital in the Home. A strong commitment to an evidence-based service to provide for consumers, carers and families requiring assessment, case management, intervention, appropriate referral and discharge planning.
