Join our dedicated Hunter Valley Transitional Aged Care Program (TACP) team and make a meaningful impact on the health and wellbeing of aged care clients. Our multidisciplinary team of Allied Health and Nursing professionals works closely with clients and their families, employing an enablement-focused approach to provide slow-stream therapy. Our goal is to enhance clients' function, independence, and safety at home following their discharge from the hospital.

As a part of our home visiting service, you will have the flexibility to deliver personalized care based on patient needs. Operating from Muswellbrook Community Health, this role offers five 8-hour shifts per fortnight with flexible days. Our service runs year-round, Monday to Friday, from 8:00 am to 4:30 pm, ensuring consistent support for our clients.

In this role you will be working as part of the Transitional Aged Care team, alongside Nursing staff, Physiotherapists, Occupational Therapists, Social Workers. You will be responsible for the assessment and care of clients to assist them to achieve their goals.
