Occupational Therapist

Working within a Multidisciplinary team, utilising your specific clinical area of expertise, to provide rapid assessment and intervention to support patients following hospital discharge and to avoid low value healthcare utilisation. This may include assisting complex patients to access health services.

The Role

An opportunity has become available for a several Occupational Therapist, Physiotherapist or Social Worker - Senior (Complex Discharge Service and Multi-disciplinary Avoidance and Post-Acute Service) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

  • High-level clinical decision-making to support patients on discharge from hospital and for patients in the community to avoid hospital through rapid, practical multi-disciplinary assessment and intervention 
  • High level, clinical services, advice and navigation for patients with significant clinical, functional, psychosocial and other complexities who receive service through Gold Coast Health and in the community
  • As a senior practitioner working in your area of expertise across all specialty areas with minimal clinical practice supervision, contribute to the development and integration of Allied Health services
  • Adapting routine clinical practice based on valid and reliable evidence to enable client focused, innovative and ethical solutions to clinical, psychosocial and external complexities

About you

To be successful in this position you will require:

  • A tertiary degree (or equivalent) in Occupational Therapy, Physiotherapy or Social Work
  • AHPRA Registration or eligible for membership to the Australian Association of Social Workers
  • Blue card (for roles not covered by AHPRA registration - GCH will cover cost)
  • Drivers Licence
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Permanent part time (30.4 hours per fortnight)
  • Base salary ranging from $62.75 - $67.53 per hour

The Integrated Multi-disciplinary, Post-Acute and Complex Care Team services teams including the Complex Care Nurse Navigator Service, the Complex Discharge Service the Multi-Disciplinary Avoidance and Post-Acute Service and the newly created Integrated Multidisciplinary Proactive Risk Identification and Navigation Team come under the overarching leadership of the Integrated Care Services, Allied Health and Rehabilitation Services, Emergency and Specialist Services.
