The Community Services Team and Rehab@Home are looking for several Physiotherapists to join their team. The community services team is a multi-disciplinary team that provides acute and post-acute care via hospital avoidance and hospital in the home interventions. Care will be provided across seven days, in various settings including hospital clinics, community centres and consumers places of residence. The Rehab@Home service is a multi-disciplinary, time limited community service providing rehabilitation, restorative care and urgent maintenance level services to clients including Transition Care Program within the Metro South Health at Home.

About the Role
The purpose of this role is to deliver Physiotherapy services within Metro South Health at Home, Community and Oral Health Services.
This includes:
• Detailed clinical assessment, patient education, therapeutic intervention and/or management and assistance with discharge planning within a multi-disciplinary context.
• Clinical education, training and supervision of students and allied health assistants.
• Participation and contribution to quality improvement and research activities under the guidance of senior staff.
