The Role: 

 

An exciting opportunity exists for an enthusiastic Home Dialysis Nurse to join the team at Northern Health.

This role requires provision of a high quality home dialysis service delivering integrated clinical, teaching and patient education aspects of the home dialysis program.

 

The role will be responsible for the training, education and ongoing management of patients in the Home Dialysis Program and working collaboratively with the multidisciplinary team in the delivery of high quality service and care. Flexibility in working days and locations and travel are required. It is anticipated that the role may evolve and change with the needs of the changing renal service.

 

To contribute to the implementation and ongoing management of the Home Dialysis Program using the model Northern Health has designed. This includes provision of a support and liaison role within The Northern Hospital.

 

The Home Dialysis Nurse role is pivotal to the quality of service delivery, training, education & research of the Northern Home Dialysis Program including haemodialysis and peritoneal dialysis.

 

Through strong leadership, the role is responsible for the facilitation and support of the home dialysis program in working collaboratively with the multidisciplinary team in the delivery of high quality service and care.  The nurse will facilitate a patient focused environment, engendering team commitment to work at optimal efficiency in a safe working environment whilst ensuring legislative responsibilities are adhered to.  Preliminary patient training will be conducted in the home dialysis centre but the majority of training will be conducted in the patient’s home. In addition, regular home visits are part of the Home Dialysis program. Travel is therefore required.

 

The home dialysis service will operate under the Home Dialysis Model designed by Northern Health which is an integrated model incorporating pre-dialysis assessment and education, patient training and ongoing support and management of patients undertaking both Peritoneal Dialysis and Haemodialysis in the home setting. The Home Dialysis model promotes optimal patient self-care and facilities the Northern Health home first approach to modality selection.

 

The Home Dialysis Nurse will play a significant role in the implementation and ongoing management of the Home Dialysis Program using the model Northern Health has designed. This includes provision of a Peritoneal Dialysis support and liaison role within The Northern Hospital. This role will also provide a liaison and support role for TNH in the care of home patients. Participation in on call for PD and home haemodialysis may be required in the future as the service expands.  Finally, dedication to supporting the whole renal team is also expected as rotations, secondments and other movements within the service may occur.

 

This position is responsible for:

  • The provision of clinical care and coordination of home dialysis patients.
  • The ongoing training and education of home dialysis nurses.

Essential

 

  • Proven leader that engages staff in the provision of outstanding health care
  • Effective communication skills including verbal, written and presentation skills
  • Strong multi-tasking and organisational skills
  • Proven ability to work effectively both independently and in a team based environment.
  • Understanding of patient flow principles, access KPIs and hospital demand management
  • Staff management skills including attraction, retention, development, performance feedback and issue resolution
  • Well-developed problem solving skills
  • Must be aligned to NH values in all aspects of the role

 

Preferred           

 

  • Post Graduate Certificate in renal nursing or equivalent/working towards
  • Adult learning experience / qualifications
  • Experience in working across large multi-campus organisations or in community
  • Understanding of rostering guidelines
  • Understanding of budgeting and financial management
  • Unit specific skills
