About the Role

Community Rehabilitation offers client-centred, goal-driven rehabilitation services to maximise a client’s independence, health and well-being following acute illness or injury. Community Rehabilitation consists of large interdisciplinary teams that work with the client to achieve their rehabilitation goals and provide home and centre-based therapy.

We have an exciting opportunity for a Part Time Grade 2 Social Worker in a Fixed Term period ending in August 2024. This position is predominately based within the Dandenong Community Rehabilitation Team; however, travel to and work at any of the Monash Health sites may be required.

A Day in the Life

The Grade 2 Social Worker is responsible for working within the Community Rehabilitation program to provide best practice client care, achieve maximal operational effectiveness, develop organisational competencies, and create strong partnerships within and outside Monash Health in collaborations with Senior Clinician Social Worker and Team Leaders. Drawing on professional expertise in rehabilitation and community practice, the Community Rehabilitation Social Worker provides assessment and intervention to clients in their own environment for example their home or local community. The Community Rehabilitation Social Worker participates in quality improvement activities within the team and larger Social Worker network.

You will be working in a variety of clinical areas including (but not limited to): neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation. Community Rehabilitation social workers work within the interdisciplinary team to provide evidence-based, client centred care. Working with a diverse range of clients, no two days will be the same! What you need:

  • Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Written evidence of eligibility for AASW membership for overseas qualifications is required
  • Current Victorian Driver’s Licence
