Our valued Care Coordinators are an integral part of the highly skilled multi-disciplinary team within the Emergency Departments throughout Monash Health. We are a well-networked team from varying backgrounds within Occupational Therapy, Physiotherapy and Social work who work collaboratively with all staff and are passionate about what we do. A Day in the Life

Working within the emergency department is exciting and rewarding! We can promise an awesome team who will support you every step of the way.This is a diverse role and will afford the successful candidate to learn and practice new interdisciplinary skills. It’s not every day you get to work in the ED and implement interdisciplinary skills across Physiotherapy, Occupational Therapy or Social work so this role is certainly unique! It will suit someone who is keen for a new challenge, pivoting from the experience you already have and developing your career in a new and exciting direction. As a valuable member of a wider team, we are committed to working with you to become your professional best. We offer clinical education sessions, training on the job and career progression to enable you to further develop your Allied Health clinical and leadership skills.

About You

You are a Physiotherapist, Occupational Therapist or Social Worker. You will possess a strong patient-centred focus, well-developed leadership and communication skills and bring a range of clinical experience. We are seeking applicants with excellent problem-solving skills and a positive, can-do attitude. The ability to work autonomously is essential.Experience in Acute Healthcare is preferred, and previous experience working in an Emergency Department would be beneficial.In this exciting position you will contribute to:

  • Providing a high standard of client assessment, treatment and coordination of services to patients, to facilitate safe, effective, and person centred care
  • Providing assessments, treatments and interventions of an interdisciplinary nature in line with the competencies that have been developed for these roles
  • Participating in the development of a collaborative inter-professional team approach to achieve optimal patient care and outcomes
  • Actively challenging traditional models of care, with the aim of delivering effective clinical outcomes in the most efficient manner
  • Work with Team Leader, Allied Health staff and Emergency Department staff to ensure workload within the team is coordinated, prioritised and fairly distributed across the team and maximises operational effectiveness

What you need:

  • Bachelor of Occupational Therapy, Physiotherapy or Social Work (or equivalent).
  • Registration with AHPRA (where applicable)
  • High level knowledge of the application of theory to practice, with skills in assessment, intervention, case management and discharge planning across the continuum of care.
  • Demonstrated understanding of how social, physical and functional needs impact an individual’s performance.
