Monash Children’s at Home provides health care services for infants, children and their families at home and in the community, who require ongoing specialised and often complex medical and nursing care. Through home nursing visits, our Clinical Nursing staff are dedicated to delivering education and support to parents and carers delivered under a range of relevant programs,
About the role
Reporting to the Nurse Manager, as a Registered Nurse, you will work largely independently, providing a safe, effective alternative model of care for Paediatric and Neonatal patients needing ongoing nursing management at home. In this way, patients spend less time in the acute setting and we concurrently uphold our commitment to patient centred care.
This role represents an excellent opportunity for suitably qualified Registered Nurses to utilise their strong relationship building skills and outcome focus in this highly satisfying role enhancing families’ independence. This is a permanent full-time 1.0 EFT working across a 24/7 rotational roster. Part-time candidates will be considered.
Planning, evaluating & maintaining a high standard of safe and effective care for infants /children
Conducting comprehensive health assessments & educating those involved in care planning direct care & discharge planning in the ambulatory setting
Ensuring timely discharge planning to facilitate patients’ safe & effective transition from hospital to home
Demonstrating your commitment to developing, promoting & progressing continuous quality improvement initiatives
What you need
Current acute Special Care Nursery &/or Paediatric Nursing experience with relevant qualifications
Previous experience in community nursing, oncology nursing, wound management or lactation, highly regarded
Current AHPRA registration
Current Victorian Driver’s Licence with access to a reliable vehicle
Demonstrated accountability for delivering high quality Nursing care
Proven ability to apply excellent clinical & critical thinking skills
Ability to work independently within a Team framework
Excellent communication, interpersonal, time management & documentation skills

Current acute Paediatric or Special care Nursery experience
Current Victorian Drivers Licence
