This is an exciting opportunity for an experienced and motivated social worker to join the Social Work Department at St George Hospital. This permanent full time position is located within the St George Hospital Transition Team.

In response to community need, the St George Hospital Transition Team has been established to support individualised, sustainable and safe care, and robustcommunication for patients transitioning from one level of care toanother, including discharge to home or another care provider inthe community.

The Transition Team Social Worker takes a clinical leadership role in supporting the complex case management of some of our most vulnerable patients. The role of the Transition Team is to ensure processes are in place as patientstransition through each phase of their care, and provide support and expertise to clinical staff in managingthe patient journey and mitigating avoidable discharge delays and representations.

The team consists of a CNC, Occupational Therapist and Social Worker with portfolios allocatedamongst the team. Currently, the role of the social worker is to primarily focus on patients on residential aged care pathways and/orNCAT processes, however the beauty of the positions lies in its ability to adapt and change focus towards emerging issues within the organisation. In the future this may include safe admission avoidance in the ED, and RACF outreach for vulnerable patients discharged into temporary respite. The successful candidate will have the skills and ability to adapt and meet the evolving service needs. The position will also retain a small caseload to provide a direct clinical service and support job satisfaction for the successful candidate. You will be part of a large, dedicated and highly skilled department working with culturally and socially diverse patients and their families. The position is also required to provide support to other areas within the hospital as required. The Social Work Department is comprised of a large, dedicated and skilled workforce providing a high level of evidence-based patient care. Our departmental wellbeing and social committee also ensures that we support ourselves as well as we support our patients and their families. The St George Hospital and Health Services is part of the South Eastern Sydney Local Health District. It is an accredited, principal teaching hospital of the University of New South Wales and is not only the largest hospital within the Local Health District, but is among the leading centres for trauma and emergency management in NSW. St George Hospital is located 14kms from the Sydney CBD with easy road links to Sydney’s north, south and west. Just a 10 minute walk from Kogarah train station, St George Hospital is very easily accessible by public transport.   1. Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for membership of the AASW
  2. Extensive post graduate clinical experience in relevant specialty area with a demonstrated commitment to ongoing clinical and professional learning
  3. Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels
  4. Ability to provide in-depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff
  5. Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature
  6. Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff
  7. Demonstrated ability to plan and prioritise your own and team work requirements effectively within agreed time frames
  8. Demonstrated experience in providing crisis interventions and the ability to participate in the After Hours Crisis On-Call Social Work Service
