About the role:

A vacancy exists in our Homeless Health Service for a highly experienced and motivated Social Worker with appropriate knowledge and skills to join our friendly and supportive team. Our progressive and innovative team provides a caring environment for our staff in their delivery of culturally appropriate, high quality patient centred care. We utilise the principles of trauma informed care and strengths-based practice. We are committed to the continuing professional development of our team members.

The Corrective Health Coordinator (CHC) provides evidence-based healthcare, support and care coordination and outreach based health care services to men with complex unmet health and support needs who move cyclically between experiencing homelessness in the City of Sydney Local Government Area and incarceration at Parklea Correctional facility. The CHC collaborates with clients, the multidisciplinary team and a range of stakeholders to address clients’ cultural, holistic health and support needs utilising a strengths-based, harm reduction approach. Service provision is person-centred and the CCC takes a lead role in the coordination of healthcare within and across different care contexts, in consultation with stakeholders. The successful candidate will assist clients to access mainstream and specialist healthcare, support services and stable accommodation to improve their health and wellbeing outcomes.

The position is full time, Monday - Friday and is based between St Vincent’s Hospital Sydney and Parklea Correctional Centre. There may be occasional early start times.

About you:

We are seeking a skilled, compassionate Social Worker who is committed to working alongside men who have moved between homelessness and incarceration, with complex health needs to achieve their goals and improve health and wellbeing. You will need experience coordinating healthcare, taking a lead within multi-agency support networks and planning care while keeping clients at the centre. We welcome applications from people who are trauma informed and strengths-focussed, with a commitment to facilitating access to healthcare for those who experience significant systemic barriers.

The vacancy is open to Australian citizens, New Zealand citizens who hold a valid New Zealand passport, Australian permanent residents and those non-residents deemed by the Department of Immigration and Citizenship to be eligible to work in Australia only.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Bachelor of Social Work or equivalent tertiary qualification, and current with eligibility for membership of the Australian Association of Social Workers (AASW) with at least 5 years relevant experience in a tertiary health or correctional setting.
3. Demonstrated expertise in providing care coordination and specialist healthcare to people experiencing homelessness who have complex co-occurring health and support needs and commitment to providing culturally safe care.
4. Demonstrated commitment to and experience working within the multidisciplinary team at a senior level, with competencies and specialist knowledge in complex care coordination, the principles of trauma-informed practice and the utilisation of a strengths-based framework.
5. High level capability and experience contributing to education, quality improvement activities, research, service development and evaluation.
6. Demonstrated supervisory expertise and commitment to both ongoing professional development and clinical supervision.
7. Computer literacy and demonstrated excellent negotiation, verbal, and written communication skills to ensure constructive and collaborative relationships with a diverse range of stakeholders, throughout various organisational levels.
8. Current NSW Driver's License
