About the role
Our goal to build 'healthier rural people and thriving communities' is the shared responsibility of all involved in care of our patients and the social worker is an integral and valued member of our team. A passion for coordinating support for patients while they're in the care of our health services is at the heart of the Social Worker.
Be part of a small and dynamic team of healthcare professionals delivering health services to our local Eugowra and Canowindra communities. You’ll be an important part of the multidisciplinary health care team and also work independently as a practitioner in the provision of the clinical service to provide high quality care.
You’ll participate in performance development to develop professional skills, take part in quality improvement activities and workplace education to contribute to improved care for patients / clients and their families. You will develop and demonstrate cultural competence and a commitment to improve the health outcomes of Aboriginal people. You will need to lead in an environment of constant change, improving the quality and efficiency of services, assisting in development of procedures and guidelines as required to promote standardised evidence based practice.
Canowindra Health Service provides acute and sub-acute in-patient services alongside emergency and a variety of allied health out-patient presentations and community nursing.
Eugowra MPS provides residential aged care beds, a Walk-In Health centre and community nursing and physiotherapy services.
This role is shared between Canowindra Health Service and Eugowra Multi Purpose Service.
