The Position

Monday to Friday Roster

Adult Community – Continuing Care Team

Providing ongoing care to people with long-term mental ill-health and their families, friends and supports.

Work collaboratively with other aspects of mental health services, including inpatient, acute, and extended care services, child and adolescent mental health services and older persons mental health services.

Complete comprehensive assessments.

Provide collaborative and person-centred treatment, care coordination, and advocacy.

Essential Criteria

AHPRA registration Acute Mental Health Experience 

Current BLS

Current National Police Check

Current Evidence of Vaccines/Immunity

Drivers license
