We aim to assist older adults to restore, improve or maintain their independent functioning and wellbeing following a recent hospital or emergency department admission or deterioration in health. This is a critical role to assist in delivering occupational therapy services supporting this objective.

About the Role
Your target client group will include frail, older people, people from culturally and linguistically diverse backgrounds and people from Aboriginal and Torres Strait Islander Communities. Working as part of an interdisciplinary team, your key activities will include:
Working collaboratively with others to provide clinical care to patients/clients and their families/carers in the community;
Applying your specialised knowledge to effectively plan, coordinate and deliver high quality, professional care and evidence-based education for patients/clients and their families and carers within a flexible service delivery model; and
Participating in research and/or evaluation of service delivery models.

You can demonstrate a sound level of clinical skills and independence in Occupational Therapy including evidence-based:
Holistic assessment of occupational performance;
Patient/client monitoring and evaluation;
Discharge planning; and
To achieve patient/client health outcomes within specified timeframes. Previous experience delivering community-based services and working with diverse populations will be highly regarded. You will also need to provide:
Evidence of current working rights in Australia
Relevant qualifications and current general registration with the Occupational Therapy Board of Australia
Eligibility for membership of Occupational Therapy Australia
SWEP registration – green prescriber status as a minimum
Current unrestricted Driver’s Licence
The nature of this role requires a willingness to travel across multiple Monash Health sites. Current Victorian Drivers Licence
Current registration with AHPRA
Ability to organise a complex client caseload
Experience working in a community environment
Experience working as part of a multidisciplinary team
