Purpose of Position
To provide comprehensice case management services to clients with enduring treatment and support needs within a recovery framework, assertively linking the client to local rehabilitation, support services and other treatment servicesas required, ( fore example, alcohol & drug services, sexual assult services, Community Health, FACS, CMO's and other community support).
To maintain a collaborative are approach with General Practitioners.
To work closely with all teams of Community Mental Health, and Inpatient Health services to provide seemless transitional care.
Have one of the following Tertiary qualifications and can obtain the criteria for each
profession as listed: *Registered Nurse (current List A - NSW Registration, AHPRA
Registered). Allied Health Professionals 1 or 2: *Occupational Therapist (registered
with AHPRA). *Social Worker (eligible for membership of the AASW). Professional
which fall under the Health Professionals State Award (Social Work and Occupational
Therapy) must provide evidence of 3 years clinical experience in their profession to be
appointed at a Level 2 (Level 3 where appropriate). *Psychologist (registered or able to
obtain registration in NSW).
Demonstrated experience of the principles of care planning and care coordination
within a mental health Assertive Community Treatment approach to care for Older Persons and/or Adults with mental health issues.
Ability to work independently and as part of a multidisciplinary team.
Demonstrated ability to develop and maintain effective partnerships with a range of
key stakeholders including General Practitioners, community managed organisations,
inpatient and community Mental Health Drug and Alcohol teams, consumers and
Demonstrated high level communication and negotiation skills (written and verbal),
and ability to actively contribute to clinical care planning processes in a team setting.
Valid unrestricted Drivers Licence for use within NSW/Australia or eligible for same.
Ability and willingness to travel for work purposes which may include travelling long
distances within the health service.
